- Emergency repsonse 24 hours a day, 7
days a week.
- Development of a Support Plan that best manages
your financial and community resources.
- Partnership with you and the important people
in your
life to realize your goals.
- Regular contact to discuss your plans, progress and
concerns.
- Coordinate purchase of necessary goods and services.
ELIGIBITY
You must be...
- A resident of Hillsborough or Pasco County
- A customer of Developmental Services
- Eligible for services under the HCBS, FSL
Waivers or CDC+
